NDIS Plan Review: How to Prepare and What to Expect

Align Network 1 November 2025

Preparing for Your NDIS Plan Review

Your NDIS plan review is your opportunity to ensure your funding reflects your current needs and goals. Good preparation can make a significant difference to your outcome.

When Does a Plan Review Happen?

Plan reviews typically occur:

  • At the end of your current plan period (usually 12 months)
  • When you request a plan reassessment due to changed circumstances
  • When the NDIA initiates a review

What to Prepare

1. Evidence of Current Supports

  • Reports from your therapists and support workers
  • Progress notes and outcome measurements
  • Invoices showing how you've used your funding

2. Evidence of Unmet Needs

  • Documentation of supports you couldn't access
  • Waitlist letters from providers
  • Letters from medical professionals supporting additional funding

3. Updated Goals

  • What you've achieved in the current plan period
  • New goals for the next period
  • How your goals align with NDIS outcomes

4. Functional Impact Statements

  • How your disability affects your daily life
  • What supports you need to participate in the community
  • Any changes in your condition or circumstances

Tips for the Review Meeting

  • Bring a support person if you'd like
  • Be specific about what you need and why
  • Use the NDIS language — talk about "reasonable and necessary" supports
  • Don't be afraid to ask for what you need
