What Does a Support Coordinator Do?
A support coordinator helps you implement your plan:
- Connects you with service providers
- Helps you understand and use your plan
- Assists with crisis situations
- Builds your capacity to manage your own supports
- Prepares you for plan reviews
Can I Have Both?
Yes! Many participants benefit from having both a plan manager and a support coordinator. They work together to ensure your plan runs smoothly.
